Thank you for your interest in participating in the Sioux City Knights Special Olympics team. Below you will learn about the necessary forms to fill out.

All forms must be submitted to Sue Davis (head of delegation) at sc_knights@hotmail.com.

Athletes

To be eligible to participate in Special Olympics, traditional athletes must be at least eight years old and identified by an agency or professional as having one of the following conditions:

  • An intellectual disability;
  • A cognitive delay; or
  • A development disability.

Participation in Special Olympics starts at age eight, and there is no maximum age limit. Before athletes can begin competition, they must submit the SOIA Release and Medical Forms, which includes:

Class A Volunteers

Class A volunteers are anyone who has direct contact with athletes; is in a position of authority, supervision or trust of athletes; or handles substantial amounts of cash or other assets for the Sioux City Knights & Special Olympics Iowa. Examples of this may be a coach, Unified Partner, chaperone, board member, Law Enforcement Torch Run member, committee member, area directors, one on one, or transport.

To begin the process, complete & submit the Class A Volunteer Application to Sue Davis at sc_knights@hotmail.com. You will be directed where to proceed & complete the application process. Further information can also be found on the Special Olympics Iowa website.
